Non-medical factors, such as isolation, can have a major impact on a person’s health, well-being and quality of life. The State Health Plan is launching its first initiative to support members beyond their medical care by connecting them with community resources that can help meet their individual needs.
Reema Health is a community-based organization that helps members navigate local resources and health care. Reema’s trained community guides, who live in the communities they serve, will provide one-on-one support tailored to help members with their specific needs, such as staying socially engaged in their community, finding a medical specialist or connecting with other applicable resources. Community guides may conduct outreach to members through flyers, phone calls, emails, text messages and in-person visits.
Starting this month, Reema, through a partnership with BlueCross BlueShield of South Carolina and supported by PEBA, will begin rolling out its services in the Midlands with a hub in Columbia. State Health Plan members, including Medicare primary members, are eligible to participate at no member cost. Five other regions will follow with their rollouts in 2025. If a member lives in a region not currently active, they can add their name to a waitlist to be called once Reema comes to their community. Expected future rollouts include:
- Greenville, spring
- Conway, spring/summer
- Rock Hill, summer
- Charleston, fall
- Sumter and Florence, fall/winter
